This article summarizes Louis H. Winer's description of the dilated pore and some of the pertinent literature about the dilated pore.
To better understand the discussion that follows, there is a brief review of the histology of the follicular infundibulum and isthmusthen a discussion of the architectural and cytologic differentiation of proliferations that reputedly differentiate toward the infundibulum.
These other proliferations with their original photomicrographs are discussed rather extensively to support the argument that they do not differentiate either architecturally or cellularly toward the infundibulum.
I then present evidence that contradicts those authors who claim that Winer's dilated pore is only a cyst and conclude that it is a neoplasm sui generis and the only neoplasm of the skin that differentiates architecturally and cytologically towards the infundibulum the infundibuloma.

Unlike your common blackhead, a pore of Winer has contents so densely packed that the pore can dilate anywhere from a few millimeters to more than a centimeter in diameter. A pore of Winer is filled with keratin and dead skin cells, which is the white substance seen extracted in this Dr. Lee video. It also has an oxidized plug that gives it a distinct black appearance. Pore of Winers can swell and become infected if prodded and picked, often making them more painful than your average blackhead.
Though the title of the video names the skin concern a dilated pore of Winer, Dr. Lee actually opens the video calling that label into question. She says the growth appears to be a cyst that has opened up a pore on the surface of the skin.
But whatever the growth may be, it is certainly satisfying to see its removal. United States. Type keyword s to search. Today's Top Stories. The 50 Most Unique Gifts for Guys. Prognosis is excellent for these lesions as they are benign and do not require any further testing or work-up. Histopathologic evaluation can confirm the diagnosis in uncertain cases. Removal can be performed via excision for cosmetic purposes. Some have considered the dilated pore of Winer to simply be an epidermal inclusion cyst with reactive hyperplasia of its epithelial lining and others proposed it to be a variant of nevus comedonicus NC.
Winer, in his original article, noted an association with a history of inflammatory cystic acne and other cystic processes. Although it occurs in both sexes, a dilated pore of Winer occurs more frequently in males compared to females and is also more frequent in whites. A dilated pore of Winer is characterized histopathologically by a markedly dilated follicular infundibulum extending deeply into the dermis.
The cavity is filled with lamellar keratin material. Radiating off of the epithelium are regularly spaced, small, finger-like epithelial projections pushing into the surrounding dermis.
These finger-like projections do not contain keratin cysts, ducts, or hair shafts. A dilated pore of Winer will present as a single, enlarged pore. Once the substances within the pore are expelled, a keratotic substance, alike the original plug, refills the pore within a few weeks. In order to expel soggy, white keratin from the deeper portion of the dilated pore, you have to repeat the motion. Many patients tend to leave the affected pore untreated, which allows for an enlarged affected area.
This condition is more likely to occur on people who have had a history of severe ache. Consult a dermatologist promptly if you have such a skin condition. The condition usually affects the face; although it is benign, many consider it to be gross and unsightly.
A dilated pore will often appear as a single, large comedo on the face, most usually on the cheek, upper lip or forehead. The condition can also affect the trunk, like the back or chest, but there are some rare incidents where a lesion on external ear carnal is found. An individual's skin around the affected pore will often not change, showing nosigns of induration or inflammation. The cause of this condition remains wholly unknown. But the primary inducement of the dilated pore may be the obstructionor infection of a person's follicle ostium, which is similar to other cystic conditions.
As for race, this condition has been reported in white males on average. As for sex, although both sexes are possible to be affected by this condition, it is more predominant in males than in females. As for ages, the condition can affect people of all ages, although most cases are diagnosed in people over However, many cases report having suffered with the dilated pore for many years before seeking treatment.
As each case of dilated pore can vary in severity and size, how to deal with the condition also varies. For minor lesions, some patients may be able to successfully treat the condition with exfoliating skin scrubs; although it is recommended to visit your dermatologist in relation to what products are the best suited to use on your condition.
Maintaining good skin care habits helps to prevent lesions appearing on your face or trunk, and reduce the occurrence. Just try to avoid any bad habits, such as scratching or picking at the affected area. This can exacerbate the condition and possibly cause the pore to become infected.
To remove the dilated pore, a referral from your doctor is necessary to make your first appointment for removal. This is usually achieved via a punch excision, around mm.
